I want to thank this teacher?

OK there there is this science teacher. She is also my homeroom teacher. And I really want to thank her for everything she did to me. I am in high school and I first moved to Canada from another country and I had bad language. Then when I went to school, I didn't know anyone and I had no friends, and this teacher was the only person who was helping me and explaining me stuff untill I finally get it. Now 1st semester is coming to end and i won't have science any more. And next year even if I don't go to another school, I still won't have same science teacher because she is grade 9 only. I like her as a person (i mean i dont love her) and I really want to thank her for everything. But I am kinda scared and I don't know whats the best way to do it. please help.
